I think I'm basically done with the [key]=val syntax.

One further possible change might be to enforce the use of that syntax
for associative (but not normal) arrays when KSH_ARRAYS is set.  That
might cause problems with some half-converted scripts.  It doesn't seem
worth a separate option just for that.
